Making a Difference...
"In those last few days she (my wife) continued to smile right through those final hours and at the end, HospiceCare was able to fulfil her wish to be at home in her own bedroom surrounded by her family."
How we help...
We deliver a person-centered Hospice at Home service across North Northumberland, offering practical and emotional care and support to adults aged 18 years and over, who are living with a terminal illness, requiring palliative input. The care is provided within a person’s home by our specially trained nursing team and healthcare assistants, 365 days a year, day or night, planned or in a crisis situation, with an emphasis on promoting quality of life.
You can access our Hospice at Home service through any healthcare professional e.g. GP, District Nurse, Macmillan Nurse, or Community Matron – just ask them to refer you.
We also provide Family/Bereavement Support, Dementia Support, and Wellbeing services as well as an Information & Advice telephone service.
